Franchisee Information Submission Form
Before we can finalize your DapperTails Franchise Agreement, we need to gather key information to complete your franchise file. This form ensures that all personal, business, and ownership details are accurate and on record with our franchise development and legal teams.
Please do not complete this form until your business entity has been officially formed (LLC or Corporation). The entity must be created under your own chosen company name and must not include “DapperTails” in the legal entity name. DBA registration should not be completed at this stage. A DBA may only be filed after the Franchise Agreement is fully executed and your official franchise number has been assigned, at which time the DBA will follow the required structure: DapperTails [Franchise Number].
All required fields must be completed carefully, as your agreement cannot be issued until this information is verified in full. Thank you!
